I would probably have to say the one that was around for years that tons of people had used before. Which is the one where a older gentlemen was bent over stretching open his behind so to speak.

When bofeity and Dr. Brown clashed on our servers a little while back they started up a "spray" war and it got pretty childish.. viciously gay porn and the likes

I finally thought of my worst one I encountered. Someone really had a grudge against me, so they went to the effort of getting a sprite of an old DooM meme that was turned into a mod, and made it an animated spray. I will only describe it as impse. Google at your own risk. (not as bad as goatse, just doomguy screwing an imp)
